These roles are part of impactful statewide programs including Free Through Recovery (FTR), Community Connect, and 1915(i) Medicaid services.

Care Coordinator – Job Overview

As a Care Coordinator, you will work closely with individuals involved in the criminal justice system or enrolled in 1915(i) services to support their mental health, substance use, and behavioral health recovery goals. Your role is to coordinate care, build recovery-oriented service plans, and ensure individuals have access to the resources they need to thrive in the community.


Key Responsibilities:

  • Engage and build trusting relationships with clients referred through FTR, Community Connect, or 1915(i).
  • Develop individualized care plans based on clients' needs, goals, and strengths.
  • Coordinate services such as housing, employment, treatment, and community support.
  • Collaborate regularly with parole/probation officers, treatment providers, and other stakeholders.
  • Maintain accurate documentation and meet program-specific reporting requirements.


Preferred Qualifications:

  • Bachelor’s degree in social work, psychology, human services, or a related field (or equivalent experience).
  • Knowledge of behavioral health, case management, or criminal justice systems.
  • Strong communication, organizational, and problem-solving skills.
     

Peer Support Specialist – Job Overview

Peer Support Specialists use their lived experience with mental health, substance use, and/or involvement in the justice system to inspire hope and empower others on their recovery path. Peer Support plays a critical role in building connection and reducing barriers for individuals navigating services through FTR, Community Connect, or 1915(i).


Key Responsibilities:

  • Provide peer-based support to individuals in recovery, fostering hope, motivation, and self-advocacy.
  • Share personal experiences in a professional and supportive way.
  • Assist with accessing services such as housing, treatment, or basic needs.
  • Help individuals build natural supports and engage in community life. 
  • Document services and maintain confidentiality and ethical boundaries.
     

Required Qualifications:

  • Lived experience in recovery from mental health and/or substance use challenges.
  • Must meet state requirements to become a Certified Peer Support Specialist (or be willing to complete training).
  • Strong interpersonal skills and ability to work independently.
