Peer Support

Peer support specialists utilize their lived experience to assist others on their journey to recovery and wellness. Peer Support offers a level of acceptance and understanding not found in other professional relationships. Peer workers walk alongside people in recovery offering assistance with goals, community engagement and strategies to live a life of hope and fulfillment.


Care Coordinators

Our care coordinators are here to support you every step of the way on your recovery journey. They provide a range of services designed to help you achieve your goals:

  • Peer Support Connections
  • Employment Assistance
  • Educational Support
  • Barrier Identification and Solutions
  • Housing Assistance

1915(i) Medicaid

All members work with a care coordinator. Care coordinators help the member create a person-centered plan of care. Members work with care coordinators to set goals. They will choose services and supports to help them achieve their goals. This helps members live successfully in their homes and communities.

Free Through Recovery

Free Through Recovery Requirements:

  • ND resident.
  • 18 years old or older.
  • If you are on probation, parole, or a transfer from prison, ask your case manager about free through recovery.

Community Connect

Community Connect Requirements:

  • ND resident.
  • 18 years old or older.
  • You need to have a mental health or substance use disorder diagnosis that greatly interferes with many aspects of your day-to-day life.

1915(i)

1915(i) Requirements:

  • Age 0+.
  • Currently enrolled in ND Medicaid or Medicaid Expansion.
  • Meet Federal Poverty Level of 150% or below.
  • Receive a WHODAS score of 25 or above.
  • Reside in and will receive services in a setting meeting the federal home and community-based setting requirements.
  • Have a diagnosis of substance use disorder, mental illness or brain injury
